According to the federal government's medicare database, of the over than 15,000 nursing homes in the country, only 31 received a perfect five star rating in the four categories - overall rating, health inspections, nurse staffing, and quality measures of individual care. Since our last check, three nursing homes have lost their Honor Roll distinction and 3 others have been added
