If you want to find out about the Civil Penalties & the last Annual Survey with the Complaint Survey that was done at the same time, go to this link. The last survey had 49 Citations. They have had 5 Civil Penalties from the State. It doesn't matter who says what in their review. This is the truth. http://hsapps.azdhs.gov/ls/sod/Provider.aspx?type=LTC

This facility is far from caring or have their residents in mind. If you care for your family member as I did, you would go elsewhere! The staff is short-handed, the administration is rude. Overall just a horrible place.

While looking for a great facility for my mother I found Oasis Pavillion, I thought it sounded great. Boy was I wrong, after seeing how things went for a week I took my mother out of Oasis and WILL NEVER send her back. The nurses are more interested in their paychecks, than they are of the patients... What a horrible place. The state needs to close them down!!!

While there were issues in the beginning, many concerns and issues have been worked out. Much of negative reviews are from disgruntled employees who are no longer employed there. The poor care was the fault of many of those talking negativley about the facility. The staff working there now all have a good heart and are working towards establishing a high quality facility. We have a wonderful, caring DON who is working very hard to Oasis Pavilion a wonderful place to be. I am a nurse at the facility on one of the long term care halls. I am proud of my hall and the care we give. Issues in the past have been addressed and many things have changes. I wish these disgruntled people would just let it go and realize many things at Oasis Pavilion have changed for the better.

Response: Tell me why the last survey had 49 State Deficiencies...http://hsapps.azdhs.gov/ls/sod/Provider.aspx?type=LTC

I could not trust my loved one in their care.
Please- ask the local health care providers in the Casa Grande area before looking into this facility. There are many issues with this home that the local community is aware of.
I can NOT recommend Oasis Pavilion Nursing and Rehab
