Fountain View Village is an average sized, for profit, nursing home with 64 beds based in Fountain Hills, AZ. At last check, the facility had 36 residents indicating that it is 56% occupied which is about average within the state of Arizona. The provider accepts only medicare, and provides resident counseling services.
This nursing home and assisted living facility, is located in a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Fountain View Village, was 4 stars, which is a better rating than 84% of nursing homes in Arizona.
We have compared the detailed Medicare data for Fountain View Village with other senior care providers in Arizona. When compare to the state averages for staffing, the number of registered nurse (RN) hours per resident per day is higher than average; Certified nursing assistant (CNA) hours per resident per day is about average; The number of licensed practical (LPN) or vocational nurse hours per resident per day is lower than average. Response: As the new Executive Director of Fountain View Village, I would like to update you on the positive changes that have occurred at Fountain View over the past nine months. We have new leadership in our skilled center with our Director of Nursing. She has grown up in the long term care industry and is committed to quality care. The Administrator has over 29 years in long term care with a proven track record of excellence. Our center is currently rated as one of the top skilled nursing communities in the United States with a five star quality rating by Centers for Medicare and Medicaid. Please feel free to stop by and visit our center and experience the difference in Fountain View!

Response: Our mother lived in FVV for several years, but we had to move her out because she was not receiving proper care. The big "for-profit" company that now owns FVV has poured millions into carpeting, paint, and other external things for a beautiful appearance. But there is high turnover of caregivers, little supervision from management, and complaints seem to fall on deaf ears. We found pills all over the apartment when moving her out because the caregivers responsible for administering medication did not ensure that meds were actually taken. If you are considering placing someone there, beware!

Response: This is an obvious plant, as similar review was posted on another site. As a former employee, I ask you to arrive around 6pm or 6am and see what happens. The assisted living or memory care are different than the skilled unit--which provides horrible care (not to mentioned the nauseating décor)
