Ugh. Expect any issue to be me with some variant of bureaucratic, officious, "not my problem" corporate medical stereotypes from the word go. Senior nurse Dahlia is eager to explain how somebody else did something they shouldn't have and how she can't really do anything about it but is astonishingly slow on remembering that there is a human being in need at the center of the problem. Read the negative reviews on this facility's rehab center and send your loved one somewhere else.

    My mother recently received rehabilitation services from Freedom Plaza. The people that work at Freedom Plaza are very friendly. Beyond that there is very little positive I can say about her experience there. In the 2 weeks of her, supposed, rehabilitation she never walked more than 150 feet. She had been labeled a liability. This is interesting, considering the entire reason she was sent there was to walk and rehabilitate after open heart surgery. She walked more in the hospital ICU than she did at Freedom Plaza. During her two weeks there she was not showered or bathed one time. She was continually pulled up out of her recliner by her arms, which is contraindicated after open heart surgery. We met with case management to address our concerns, but none of them were addressed. Case management rarely returned phone calls. I would not send my mom back to Freedom Plaza given the choice. I would not recommend that anyone send their loved one there for rehabilitation. They may do a good job at long term care but they do not do a good job at rehabilitation.

    Freedom Plaza Care Center is an average sized, for profit, nursing home with 111 beds based in Peoria, AZ. At last check, the facility had 73 residents indicating that it is 66% occupied which is about average within the state of Arizona. The provider accepts only medicare.
    This nursing home is located in a continuing care retirement community (CCRC). Important to note that this facility has recently improved and has gone from 2 stars to 3 stars.

    My qualifications. Son (engineer, manager, business owner) of senior parent at Freedom Plaza Care center. My senior parent has been in and out of “rehab” facilities over the years so I have high level visibility on how said facilities functions.

    While at Freedom Plaza my senior parent fell a second time and cracked pelvis and broke two main wrist bones. It was on the second fall said parent broke the bones. At competitor(s) rehabs, my observed experience is, said patient is a fall risk the thus 100% attention must be given to said patient. My senior parent said the aid stepped away just before second fall. This has not been verified. (Fact: senior parent’s body is giving out but brain is still sharp.)

    On another matters, staff hardly ever called back. The floor doctor at Freedom Plaza NEVER called back. Social worker never called. Case manager never called. I was unable to get a handle on care of my senior parent. When I asked for a list of meds, an unreadable hand written list was provided.

    I do give an A+ to the nursing staff - RNs. Their care and interact of senior people is fantastic.

    My interaction with management was limited. If I had to ascertain where the problem(s) lay at Freedom Plaza I would say it’s squarely a poor management situation.

    Response: I agree with this statement, Freedom Plaza is a poorly run Hospice facility. Run by people who have no idea about management, comunication skills and are very rude.
