Arizona Grand Senior Living Community is a small, not for profit, nursing home with 36 beds based in Phoenix, AZ. At last check, the facility had 30 residents indicating that it is 83% occupied which is about average within the state of Arizona. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
This nursing home and assisted living facility, is located in a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Arizona Grand Senior Living Community, was 4 stars, which is a better rating than 84% of nursing homes in Arizona. Important to note that this facility has recently improved and has gone from 3 stars to 4 stars.

Arizona Grand is a beautiful facility located in the biltmore area of Phoenix which is a great location.

My experience with Arizona Grand's skilled nursing center was very, very bad. Nurses are well-intended but woefully understaffed. CNAs are frequently apathetic and/or lazy. Communication between staff members is poor. My loved one was at AG recently and had to be transferred to another facility due to inadequate levels of care. Be warned.

My mother was in the care Center and then transfered to the assisted living unit. The staffing was so poor she was left alone several times and fell twice. The last time fracturing her spine and required transfer to the hospital. We have been harassed since with bills from the AZ Grand. THey claim she broke her contract by transfering out of the facility. As though she planned to fracture her spine.
