I went there after knee replacement for rehabilitation. I quickly realized I had made a mistake. I checked myself out two days later. Poor care, cold food, understaffed, unresponsive most of the time. I would not recommend this facility for anyone anytime.

    If I could I would give them negative stars, they don't deserve any stars at all. DO NOT let someone you love stay at this place. I wouldn't even want someone I hate to be at this place. Not enough staff to give any type of quality care. A staff member told my mom that she didn't have time to get my grandmother off the commode. My grandmother had just had 3 screws put in her hip and was hurting. They left my grandmother on the commode for over 35 minutes. My grandmother got a UTI and it was so difficult to get a doctor to see her. Her infection was so bad that she was slurring her words. My grandmother died choking on her dinner because they had her sitting in an odd way, but she had orders to NOT leave her alone while she was eating.
    My grandmother was at this horrible place for only 4 days. We tried to be there all the time, but my mom had just left an hour before she died. My grandmother died August 3rd. The nurses notes wrote in their report that my grandmother woke up in good spirits on August 4th. My grandmother was a strong woman and she was only there for rehab for her hip that she had broke. Please don't let this happen to someone you love. The staff wrote that she woke up in good spirits, when she had died the night before.

    Las Fuentes Care Center is an average sized, for profit, nursing home with 128 beds based in Prescott, AZ. At last check, the facility had 86 residents indicating that it is 67% occupied which is about average within the state of Arizona. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
    This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Las Fuentes Care Center, was 4 stars, which is a better rating than 84% of nursing homes in Arizona. Important to note that this facility has recently improved and has gone from 2 stars to 4 stars.

    All I can say is that if you love your family member do not...DO NOT subject to this care facility. My dad was here for about 10 days and it didn't take long for his health to decline. In the 10 days that he was here he lost 25 lbs. The food was always cold and smelled like cat food. My dad was totally dependent on the staff and yet on only a few occasions had his needs met. We were told that the policy stated that the resident be changed every two hours. We would spend entire days at the facility and not once did someone come into the room to change him or even check him. His hand was injured and no one could tell us what happened. Nothing was documented. I dont know why this facility is still open. I dont understand why with the awful ratings that it has that they are still able to be paid good money to slowly kill people. It is the saddest thing I have ever seen. You are not a human being with feelings here. You are not a person deserving of respect here. Everyone deserves respect and love so do your family member a favor and choose another care home. Hopefully one that actually lives up to the name of CARE home.
