my mom was there and was treated like a dog.the management is a total joke and nobody is qualified to run the place , they dropped my mom several times and was denied physical therapy all the time because they were short staff or the equimpent was broke then they said she was not trying to improve and just keep taking our money and gave her sleeping pills so they would not have to care for her and they would never give her a bath or they would let her sit in her @#$% for hours before cleanning her up i do not recomend this place to anyone

A small for profit one story facility. The facility was very clean with no smells of urine, feces or body fluids.
It is a very low key environment. The rooms are large with large bathrooms. Quite enough linens and towels etc. The showers are large and well equipped. The Activity room is large. The activities are appropriate and the activity director seems to like her job. They have a beauty shop and a little store for purchase of sundries. There is a safe courtyard in the middle of the facility with ease of access. This adds a very nice place for visits . The main dining room was clean and large. decent tables and chairs.
The staff members were welcoming and attentive. There is a focus on safety . The beds are equipped with alarms to alert if the patient gets up . The beds are low and they put a mat on the floor as well. The food is adequate, there are substitutions of burgers, salad, fruit etc. The therapists are ok. They accept medicare and various medicare advantage plans.Of the six places we evaluated this was the ONLY one which was actually clean, one story, decent reviews (there are several websites with reviews), in a reasonable location and was in good standing with medicare and the State Dept of Public Health. We were pleasantly surprised at the consistency of cleanliness and quiet regardless of time of day. There were no issues with medications or getting care or therapies ordered. We are not sure how the facility would stack up in a long term care situation but for short term rehab (PT and OT) care, it was very good.

Please think twice about choosing Avalon Care Center Shadow Mountain. My Grandmother has been neglected from day one. I fear and worry for not only her safety and well being but for all that are there.
My hope is to prevent others from putting their loved ones in what my opinion is a dangerous facility.

Avalon Care Center - Shadow Mountain is an average sized, for profit, nursing home with 120 beds based in Scottsdale, AZ. At last check, the facility had 84 residents indicating that it is 70% occupied which is about average within the state of Arizona. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Avalon Care Center Shadow Mountain, was 2 stars, which is a lower rating than 65% of nursing homes in Arizona. Important to note that this facility has recently improved and has gone from 1 stars to 2 stars.

This place doesn't clean the rooms and the nurse ( some of them ) are clueless. You have to wait for your meds and they give them to you when they want. They do not follow up on orders as per the doctor, so you have to keep asking and asking. And that doesn't work either. They lie to you when you first visit and promise the world only to lie behind your back. I wouldn't leave anyone there if you are not going to be their daily to check on them to make sure they are being taken care of. This place is full of neglect.
