Very disappointed. My dad was placed in this facility for rehabilitation after his hip surgery, when he first arrived he was in his room for at least 3 hours before anyone went to check on him. My dad is a high fall risk due to his hip surgery, and this was mentioned several times to the staff which was ignored. He fell his first night at the Maples and this could have very well been prevented. His medication was never clarified with the family which caused him to be overly medicated and non-responsive when he was taken to the hospital. I would not recommend the Maples to anyone. We were very disgusted and upset over this situation, just glad my dad is out of this facility and is now in good hands.

The Maples at Har-Ber Meadows is a large, for profit, nursing home with 140 beds based in Springdale, AR. At last check, the facility had 126 residents indicating that it is 90% occupied which is about average within the state of Arkansas. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for The Maples At Har Ber Meadows, was 1 star, which is a lower rating than 84% of nursing homes in Arkansas. Important to note that this facility has recently detriorated and has gone from 5 stars to 1 stars.
We have compared the detailed Medicare data for The Maples At Har Ber Meadows with other senior care providers in Arkansas. My mother was at The Maples for rehab for 1.5 weeks. In that time a nurse threw away her medicine because she didn't check the spoon it was on (dosing with a bite of yogurt). This was just the time I happened to be there... made me nervous about the rest of the dosings.

The whole time she was there she was receiving only 500mg of an anti-seizure medication, a huge difference from the 2250mg she was SUPPOSED to be receiving. The person who checked her in apparently couldn't read the paperwork sent from the hospital mom came from, and it wasn't caught until the day before mom had two seizures and became unresponsive. Again, I caught the discrepancy, but by the time I was able to bring it to their attention, it was too late. Mom died in hospice two days later. The death certificate listed cause of death as "brain tumor" because that was the overall issue mom was dealing with, but she was at the Maples to rehab so she could go home and continue her chemotherapy regimen. She never got that chance due to the incompetence of Maples staff.

This facility was great. My mom was suffering from dementia, she happened to be rough at times, and couldn't walk. Out of the time she was there she never had a bed sore, or serious complications. The facility was always clean, updated, smelt good, very welcoming. The staff was great, she loved em most of the time (exceot when they woke her up for blood pressure). This was comforting even when I stayed the night, the staff would be informative, and sweet, I just wish I could have htose days back..
