About Country Meadows Retirement Communities
The motto of Country Meadows Retirement Communities is "Life gets better." That is in keeping with the underlying philosophy that exists at each community to understand the needs of its residents and to help them improve their lives. That philosophy dates back more than 50 years when former Pennsylvania Governor George M. Leader and his wife, Mary Jane Leader, founded Leader Nursing Centers. In 1982, the family started Country Meadows personal care and independent living facilities for seniors. The company remains family-managed and 100-percent family-owned to this day, with George and Mary's sons, Michael and David, serving as Country Meadows as CEO and COO respectively.
Country Meadows, based in Hershey, Pennsylvania, adheres to eight Foundation Principles that guide its employees: (1) promoting a genuine sense of community; (2) honoring the rights and independence of seniors; (3) supporting wellness in all of its dimensions; (4) encouraging lifelong growth and development; (5) providing leadership to the team; (6) practicing professional and social responsibility; (7) creating the standard for quality; and (8) establishing a culture of integrity.

There are 10 Country Meadows communities, nine of which are located in Pennsylvania: Allentown, Bethlehem, Hershey, Lancaster, Mechanicsburg, South Hills of Pittsburgh, Wyomissing, York (South), and York (West). The 10th community is in Frederick, Maryland. Two new communities are coming soon in Gettysburg and Easton, Pennsylvania. There are a broad range of senior living choices, including independent retirement living, assisted living and personal care options, memory care programs, rehabilitation and restorative care, as well as the Country Meadows At Home service that provides assistance to seniors while they remain in their own homes. These options are personalized to the individual, with the emphasis on providing seniors with security, comfort, and a myriad of lifestyle choices to engage the mind, body, and spirit.
Senior Living at Country Meadows
Country Meadows communities offer residents all-rental apartments without the expense of a buy-in, providing affordable senior living with a wide range of amenities. There is one monthly rental fee that covers the residents' apartment, utilities, meals (per their level of care), social programs, transportation, cable television and many other benefits. Plus residents don't have to choose between this lifestyle and their pets, as Country Meadows is a pet-friendly environment.
Residents' lives are enriched in body, mind, and soul by the Community Life programs that allow them to be socially active, participate in a wide range of enjoyable activities, and maintain their health through senior-specific fitness programs that support Country Meadows' culture that emphasizes a wellness-based approach to life.
Independent living is the ideal choice for active older adults who want to enjoy social opportunities while being free from the burdens of home maintenance in an environment that offers many opportunities for social interaction, recreation, and group activities. Independent living residents enjoy the comfort of their own private apartments, available in a variety of floor plans, combined with all the services and programs that Country Meadows communities have to offer. Maintenance-free living includes weekly housekeeping and laundry services. Additionally, all independent living residents receive daily continental breakfast, one main meal per day, as well as drinks and snacks available whenever they like. Independent living gives residents the freedom to pursue their own interests but also provides the peace of mind of emergency response in the event of illness.
Independent living apartments are located at Country Meadows communities in Allentown, Bethlehem, Hershey, Mechanicsburg, the South Hills area of Pittsburgh, Wyomissing, the two York locations, and in Frederick.
Assisted Living and Personal Care
Country Meadows offers Assisted Living and Personal Care services at all 10 of its Pennsylvania and Maryland communities, with residents enjoying the privacy and amenities of apartment-style living with weekly housekeeping and linen services. Country Meadows creates a specialized plan tailored to individual residents to meet their needs. Personal Care services are available to residents who need individualized assistance with daily activities such as bathing, dressing, or medication reminders. As part of Personal Care services, residents enjoy three delicious chef-prepared meals each day, snacks and beverages as desired throughout the day for themselves as well as guests, plus personalized assistance provided around the clock by a caring, trained staff. Active Community Life programs put the focus on the residents' total wellbeing with physical, social, and spiritual activities. Communities also arrange transportation for appointments, errands, shopping, and church services.
Connections Memory Support Services
Country Meadows is a leader in providing personalized therapeutic care for those suffering from memory loss caused by Alzheimer's disease and other forms of dementia. The innovative Connections Memory Support Services program is designed to provide ongoing support to residents facing memory loss, whether the effects are mild or more severe. Country Meadows Connections Memory Support Services gives residents the special support they need. For peace of mind and safety, there are also specially designed, secure living areas that are staffed with highly trained caregivers. The program offers a range of options designed to maximize the comfort and social integration of all residents in the program. This program is offered in a variety of settings, including the Personal Care communities, in secure neighborhoods and in Country Meadows' skilled nursing centers. Country Meadows can even provide non-residents with daily care through Country Meadows At Home.
As a means of helping caregivers in caring for loved ones with memory loss, Country Meadows has partnered with the Alzheimer's Association to offer monthly Memory Support Groups at all 10 of its communities. The goal of these groups is to give families extra guidance and support in caring for their loved ones.
Pathways Restorative Services(tm)
County Meadows' Pathways Restorative Services meets a wide range of rehabilitation and restorative care needs with the goal of improving and preserving the resident's independence. The highly trained Pathways care teams provide short- and long-term assistance to people who need more support than found in Country Meadows' traditional personal care setting, regardless of whether they are recovering from surgery or an injury or need help in managing a chronic condition. The range of services encompasses physical, occupational, and/or speech therapy; fitness programs to improve strength, balance and flexibility; dining services to aid in overall nutrition; specialized approaches to accomplish daily tasks; and emotional and spiritual support.
Therapeutic activities help to energize and strengthen residents so that they can improve their performance with such daily activities as bathing, dressing, and walking. As an important part of the therapy process, the highly trained Pathways team is on hand to assist and celebrate every achievement. This program is available in the Country Meadows' Pennsylvania communities in Allentown, Bethlehem, Hershey, the South Hills of Pittsburgh, Mechanicsburg, Wyomissing, and the two York locations, and also at the Frederick, Maryland, campus.
Additionally, Country Meadows offers skilled nursing as well as 24-hour, Medicare- certified care at its campuses located in Bethlehem and the South Hills area of Pittsburgh.
Country Meadows at Home
Country Meadows at Home provides personal care, homemaker, and companion services, to older adults in the comfort and familiarity of their own homes-whether for a few hours per week or for consistent, around-the-clock care. The program also includes Connections Memory Support Services that are provided to individuals with memory loss. Respectful, fully trained caregivers provide non-medical help with such daily tasks as bathing, dressing, managing medication, and meal planning-all at the same level of compassionate care offered to Country Meadows residents. Homemaker and Companion Services are available for seniors who need assistance with small jobs around the house, light housekeeping, meal preparation, shopping, and errands. In some cases, caregivers can simply provide companionship for a few hours a day or relief for a primary caregiver. In addition, transportation services are available for doctor's visits, trips to the pharmacy or grocery store, or other types of errands.
