Upon entering this facility, it smelled of urine, feces and disinfectant. This has not improved over time. Food served cold or lukewarm at best; this improved for a few weeks after commenting to Patient Advocate, but declining again. Though they clean the hallways daily, the wiping down of individual rooms is limited; showers available only once every 10 day. Facility is old and in poor condition. This facilities redeeming factor is the staff, they are friendly and make a point to greet their clients even if not working. Staff is stretched thin.
