I've been working at Lemay Avenue Health and Rehab for over a year. I've also been temping at other facilities in the area for 6 mos now.
As far as Long Term Care(LTC) goes, Lemay is probably the best. If you have a loved one that has health issues that warrant 24 hr care Lemay is clean, has good food and cares for their residents.
Our Memory Care is well staffed and good to our residents but could use updating as there is a lot of competition that, frankly, is better.
Our Rehab isn't great however and I personally think it's not a good idea to send someone to a LTC/Rehab hybrid facility because, really, how much better do they want you to get when they have a LTC room available. I also believe the LTC side of it gets in the way of allowing our Rehab patients the time they need to be more independent, we tend to baby them so that we can move on due to time constraints.
Transitioning to a facility is hard all around. Privacy, dignity and independence are all lost in some way. It's the nature of the beast. Low Census and staffing are always an issue in healthcare too. Even though I work in a facility I'm a big believer of in-Home Care as much as possible.
Long story short I think Lemay Avenue Health & Rehab is great for LTC and maybe should consider JUST doing that. Look into other Memory Care Facilities, Assisted Livings and Rehabs.

    Lemay Avenue Health and Rehabilitation Facility is an average sized, for profit, nursing home with 130 beds based in Fort Collins, CO. At last check, the facility had 126 residents indicating that it is 97% occupied which is more than average within the state of Colorado. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
    This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Lemay Avenue Health And Rehabilitation Facility, was 5 stars. Only 18% of nursing homes have 5 stars in Colorado. Important to note that this facility has recently improved and has gone from 4 stars to 5 stars.

    Some of the staff in this facility seem qualified and appear to care about residents, some clearly don't. I've watched as staff talk on their cellphones and completely ignore residents under their charge, even when they need help. They informed me after admitting our family member that outside nursing visits are required weekly, which adds $135 a week to the charges. Anything residents need, including nail clipping, haircuts and wound treatment cost extra. I continue to receive cryptic invoices for medical services (like x-rays) from outside facilities that are sometimes thousands of dollars. Communication to residents' family members seems poor here, worse than our previous facility. For example, a staff nurse put our family member on antibiotics without letting a family member know and without checking with his doctor first. We had to argue with them to get this stopped. All meds must be purchased from their own pharmacy, which our insurance won't cover. And their pharmacy is expensive. The place is clean, but with all the nickel and diming that goes on here, the monthly bills can exceed $9,000.
