This Nursing Center has declined due to extreme turnover in nursing and weak Leadership, bad operational decisions, the closure of its Secure Dementia Unit, loss of PT and other Therapy staff, and an overall bad work environment. A few years ago, this facility held a 4 star rating, was well managed and profitable, and had one of the best health surveys with a perfect Life Safety survey. Since that time, 4 Admins and 4 DONs have come and gone with each a decline in overall performance due to low experience and knowledge. Now, one their 5th E.D. and DON, the facility struggles to hold a low 70's census, constant turnover, loss of the entire PT department and the SLP and overall low morale throughout the nursing staff. Neglect and abuse is raising its ugly head.

    I noticed the reviewing rating was from 2012, and that rating was mainly based on the hard work and dedication of a long gone Director/E.D. Well, since that time, this facility has declined into the trash bin, as many caring and friendly staff left under the new replacement management. Harsh and unfriendly treatment, short staffing, less than knowledgeable leadership and inexperienced nurse managers dragged this operation into the dumps. The two 2015 surveys have been horrible with multiple G level deficient practice ratings, dirty and under staffed facility, poor attitudes and a newly licensed and inexperienced Executive Director. By the way, this is the third Director in 3 years!! Hope this young kid can perform better than his first two surveys indicate, for his census is 70 for a 62.5% rating.
