Azalea Trace Nursing Center is an average sized, not for profit, nursing home with 110 beds based in Columbus, GA. At last check, the facility had 104 residents indicating that it is 95% occupied which is about average within the state of Georgia. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Azalea Trace Nursing Center, was 1 star, which is a lower rating than 80% of nursing homes in Georgia. Important to note that this facility has recently detriorated and has gone from 2 stars to 1 stars.

We have had several family members st Azalea Trace and apppreciate more than they can ever know and that words can express, the care and love our family members received.

When I walked into the nursing home I was greeted by several of the residents. They smelled very will and had on clothes that was clean. As I continue to look through this facility I came to the first floor nursing station, and there I was greeted by several of the nursing staff with big happy smiles on there faces, which lead me to think this has got to be the best place to put my love one. I asked can I get a tour of the facility if anyone has time and they replied the social worker is the one that gives tour, but since you're here on of the cna's will walk you through the facility. I was walked through by a cna who had been there for 11 years every resident downstairs she could tell you a funny story about as if she was their famliy. After walking and talking to this cna my mind was made up I was going to put my love one there, and we are still t Azalea Trace right now and they are all still smiling. Thank You Azala Trace Staff!!!!!!!!!!!!!!!!!!!!!

this is the best facility in columbus, georgia. I was so impressed with the care my mother received, the staff involved me and my family in every aspect of her care. Columbus Hospice was in on her care in the end and the compassion shown to my family was beyond words. Why do people try to destroy this nursing home

I know who commented too; and said 'shame on me'. One has to give Azalia Trace 30 days notice to leave, then find another nursing home to take her. None here will hold a bed for thirty days. She also would have to come from a three day hospital stay to even have the slightest chance to be put into another nursing home. I told Hospice to put her here, with me. My only reply from them today was that I could see her until she was passing. I could not see her Mother's Day. I was told by Hospice that I could see her only as she was passing. This was not true. After finding out that there actually WAS an administrator to this Azailia Trace, I spoke with him today. So there WAS a meeting planned after I had been told by Hospice there would NOT be. I am now allowed to see my mother; but can no longer give her vitamins nor bring any cleaning materials from my house. There IS an adminstrator there; who DID listen to my concerns. For the vitamins, the cleaning materials, and writing to this site, I was 'properly' punished. The administrator does have his hands full. He was not happy with my comment about Azalia Trace, but is trying to change things. I was never told he existed. Nice to meet him today. He was not happy that I called the police Friday evening either. He did ask my side of the story. I thought he could back up the camera and audio to actually SEE what took place. No such luck. As the nursing administrator told me, they cannot control what rats crawl out of the sewer on Talbotten Road. Perhaps not; and the garbage outside looks better. I was asked who my doctor was and what medicine I take. That definitely does break the Hippa laws, but I conceded. I still do want to see my mother. These people never told me that there was a Doctor who administered over this nursing home; there is..his office is not a linen closet either. He does exist. He genuinely seemed to care about my concerns. I told him that I would amend the comments I had put here, as he is aware of my concerns. What a fine job they did at hiding him and telling me that two other people did HIS job. I will do anything to see my mother; but to be honest, this man does seem to be concerned about the issues there. It was NICE to finally meet him. He may find that he has 'other' issues to deal with than just 'me'. He was polite and businesslike. I told him I would NOT put his name on this site. He was or still is a psychitrist. I do think that he cares about my concerns. Shame on me? I know who wrote this. Same on YOU. I will see to it..that the administrator sees a copy of what YOU wrote.
