As a resident family member and longtime employee of Heritage Square, I feel the need to respond to all the false accusations toward Heritage Square and the administrator, Bonnie O'Connell. I have worked over 20 years in the healthcare field as well as at other nursing homes and toured many facilities related to my job. I knew there would only be one facility I'd choose when the time came to admit my mother. Heritage Square is the best for miles & miles around. My Mother loves it here & states she feels like she's on vacation. Everyone is so helpful, the food is great and so many choices on the buffet, rooms are kept spotless, great nursing care (Mom has never been healthier, after a year of falls, health concerns & numerous hospital stays.) Heritage Square is & always has been an excellent facility & Bonnie is one of the most resident oriented administrators I have ever known. She responds daily to all & any concerns with any resident whether big or small, whether a health concern, someone needing a haircut, or a ride to a Dr. appointment, or just some T.L.C. She is always available for each & every resident and for family members with any concerns, days, nights or weekends. We would love to give anyone tours to prove how rediculous all those false comments are & especially the ones about Bonnie. It's so sad there are people so negative, making these statements online & so unfair to such a great facility and administrator.

Wow…sounds to me like a few people are salty. I would like to start by saying Heritage Square is a very pleasant and respectable place to live. I believe that is solely based around the staff and administrator, Bonnie O’Connell. How dare someone try to run her into the ground…unless you’re upset you got fired for your pathetic work ethics and skill? Bonnie’s priority is the resident. If you are a family member who is just waiting for your “loved” one to pass so you can reap the benefits after, Bonnie will see it and protect her resident. Heritage Square is the resident’s home…nothing is mandatory. Forcing and encouraging residents to eat, walk, take medicine, etc. are two different things. Haven’t you ever heard, “you better use it or you’ll lose it.” It is Heritage Square’s mission to keep residents independent as long as possible and to ensure dignity is respected. If any accommodations are need (bigger bed, meal times, etc.) it is the resident’s right to voice that concern. That concern will then be addressed and resolved in a timely matter. I would be interested to see what the residents of Heritage Square have to say about their home. Every time I have been there, I see happy faces, continuous activities, pretty decorations, and a staff who views Heritage Square as the resident’s home.

These comments are absurd and malicious... I have worked in healthcare for decades in many facilities and this is by far the best facility I have ever worked. I don't know if nurses here agreed or not, but they should consider employment elsewhere and see what it is like in other facilities.This is a pretty pathetic way of communicating and destroying another human being ...

Heritage Square is a small, not for profit, nursing home with 27 beds based in Dixon, IL. At last check, the facility had 27 residents indicating that it is 100% occupied which is more than average within the state of Illinois. The provider accepts only medicare, and provides resident and family counseling services.
This nursing home and assisted living facility, is located in a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Heritage Square, was 4 stars, which is a better rating than 84% of nursing homes in Illinois. Important to note that this facility has recently detriorated and has gone from 5 stars to 4 stars.
We have compared the detailed Medicare data for Heritage Square with other senior care providers in Illinois. When compare to the state averages for staffing, the number of registered nurse (RN) hours per resident per day is higher than average; Certified nursing assistant (CNA) hours per resident per day is higher than average; The number of licensed practical (LPN) or vocational nurse hours per resident per day is about average. The most recent health inspection was on 07/26/2012.
