The facilities are nice and clean with no "smell" as many facilities have. My Aunt has been a resident for about 6 months and the main complaint we have regards the food, or what they serve as food. The menu sounds delightful but what actually arrives on a tray at meal time is often not identifiable. I think it is a shame that our elders, who have little to look forward to in their day but food are forced to eat non food stuff. Their meals come off a truck and warmed. My aunt does not eat sandwiches, and has stated as much but most lunches she is served are sandwiches. The measurement of health is measured frequently by weighing and blood samples. When a resident looses weight they are told they need to eat. I know many times my aunt has family members bring food. I have tasted her meals many times and it truly saddens me that the residents, mostly women who have throughout their lives have prepared wonderful meals for their families and church functions are forced to eat non food for the rest of their life.

    My mother was in GW skilled nursing for nearly three months before she passed away. I am glad she was in an environment that was a home-like setting and not institutional. Each 'villa' houses 10 patients in private rooms with private baths; every room with nice sized windows. The center of the building is a large open kitchen, dining area, and living room with fireplace and large screen television. Each elder furnished and decorated their own room any way they wanted which made it even more their personal space. The meals were freshly prepared in each villa by staff and served family style at the large dining room table. Every time I brought my mom back from a doc visit around meal time, I was invited to join in so I know from personal experience the food was very good. I also loved that the front door to each villa was kept locked so no one could walk into the building who had no business being there. I and my family found the staff friendly and willing to help when called upon and we liked the emergency alert pendant each elder wore around their neck that would summon staff at the touch of a button. The bricked in patio off the back of each villa is landscaped and very peaceful with a grill the staff told us we could use if we wanted.
    There were only two things I wasn't happy with while she was there: 1) the registered nurse oversees two villas at a time and sometimes when mom needed pain medication, we had to wait on the nurse to finish rounds in the other building before she would come over; 2) someone stole half of my mother's jewelry she insisted on keeping there. This could have been another resident, a staff member or someone visiting another elder. We don't know and we'll probably never know but this type of thing happens at ALL nursing facilities and wouldn't have happened if she had kept her things in a small safe or something since she wanted to hold onto them and didn't want to lose that part of her.
    Overall I'm glad mom was there her last few months and only wish she could have lived there longer as her death was sudden and unexpected. It's the type of place I would want to be if I had to live in a nursing home.

At last check, the facility had 30 residents indicating that it is 60% occupied which is less than average within the state of Indiana. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.
    This nursing home and assisted living facility, is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Villas Of Guerin Woods, was 5 stars. Only 14% of nursing homes have 5 stars in Indiana.

    Guerin Woods does not welcome Hospice and in fact made it difficult to follow Hospice suggestions and doctor's orders. Some of the nursing staff were inexperienced with end-of-life issues.
