It seems from the countless reviews of this facility that there are a wide range of issues, which leads me to believe that my problem with Michiana Health and Rehabilitation isn't the only one of it's kind. I have yet to speak with anyone at MH&R so that this issue maybe resolved. "I recieved a explanation of benefits letter from my insurance provider(which explains a claim submitted by a provider)." This letter states that I was at MH&R on March 1, 2014 and a claim was submitted to the company that I have health coverage with. The claim is for room, board and four Ancillary. Over 5000$ of services the claim is for. I have never been, nor have I heard of this facility. I hope that I get this issue resolved and it is not the start of more issues.

    I read many posts on here but saw that most of them were not recent. So I decided to tour the facility myself and see if it had changed. Indeed it had, after a very pleasant and informative tour I decided to have my mother come here and it was a great decision. The staff is very caring and informed about every condition that was going on with my mother. I would definetely recommend this facility to anyone in the area for a great place to come and get stronger be well cared for.

    The comments from a past resident/family member are spot on. With the current administration your family member would probably get better care is you sent them to a kennel. The previous administration actually cared for the residents and staff. In June of 2011 the corporate buzzards swooped in and fixed things that were not broken. It went downhill fast. There were 3 primary consultants and the right hand didn't know what the left hand was doing. One would come in and implement something and then another one would change it only for the third one to come in and wonder why we were doing things that way. Since then the Indiana State Department of Health has been there numerous times investigating complaints. I heard their annual survey wasn't that great either but those results haven't been posted anywhere as of yet. The current administrator is nothing more than a money grubbing, uncaring, evil woman. The current director of nursing isn't much better. She came from a problem facility and strives to create the same problems at Michiana Health and Rehab. The staff that was there under the previous administration we very caring and there were plenty of RN's, LPN's and CNA's to provide proper care for your loved one. The current administration figured that type of staffing cut too deeply into the profit margin and so staffing was cut. The place is a snake pit these days. I wouldn't put my pet in a place like that. The biggest downfall of this facility is the top corporate people at Extendicare. They are a realty company and probably should have stayed in that line of work instead of dabbling in the health care world. I am a RN and I left that employment voluntarily. I would rather be homeless and eat in a soup kitchen than work for a company like that. If you care for your family member at all… put them in some other facility.

    Response: Well said. I also worked there and the DON did not introduce herself to staff or observe staff or patient care. Yes, I also was advised she had worked for a previous facility that had a poor rating. The DON and Administrator fired staff with out good cause. I seen some very caring and hard working staff leave that facility.Some were terminated and some left voluntarily. The staff was cut back dramaticaly. Staff hours were cut. No consideration was made of the patients level of care needed as post op patients. It was suppose to be a "Rehabilitation Center". Post op patients and our elderly cannot be hurried. Staff was running around doing their best. Most of the time not taking brakes or lunch. How can patients recieve good care under those conditions? I felt there were so many comlaints to the State they fired Staff to give the appearance of fixing/addressing the complaints and the facilities deficencies. As I commented the DON or Administrator were not walking the units and observing.Its all about profit and making the numbers look good for the corporations report.They didn't care about the facility,the staff or patients. It's just a paycheck to them.I was sorry to see the prior DON leave. She had high expectations for the center. She wanted to make it an exceptional facility.

    This facility was not prepared to support my mother in recovering from hip replacement surgery. She was not allowed a walker, there was no toilet riser, and in general the staff was unaware of the requirements of her condition. And they did not have her medication available for more than 12 hours after admitting her, she was is so much pain she spent most of her time weeping.

    Michiana Health & Rehabilitation is an average sized, for profit, nursing home with 100 beds based in Mishawaka, IN. At last check, the facility had 73 residents indicating that it is 73% occupied which is about average within the state of Indiana. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
    This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Michiana Health & Rehabilitation, was 2 stars, which is a lower rating than 61% of nursing homes in Indiana. Important to note that this facility has recently improved and has gone from 0 stars to 2 stars.

    Well, unfortunately time has not improved this facility. I think it is time to tell the management corporation to get out. Residents continue to come last. I had a family member that was unfortunate to have stayed at Michiana recently and had nothing good to say. The food continues to be a problem! That only compares to overworked incompetent staff.

    Now that they got rid of the current administrator and DON, the place should improve. Unless, of course, they hire people like the two that just left. But, as long as Penny (Corporate Nurse) is allowed in that building, they probably won't get anybody worthwhile to stay. The corporation also needs to get rid of Teri Fowler, another corporate (who knows what she actually does except screw things up) person.

    Absolutely beautiful building from the outside but all you corporate folks need to realize it is not about the looks. Appearance, important on a scale of 1 to 10, I would say a 5. However, how a building is ran speaks volumes for the overall happiness of your staff, which effect care. It is obvious your staff are not happy I have never heard a fine word about your administrator. A matter of fact, I would have agree with them! Can I have a job that I just sit behind a desk, never acknowledge a family, a resident, or does not care what goes on? At least I have personality without a license but I can get one out of a cracker jack box because that is were she probably got it from. Concerns? Does she even know what the word means? Wake up you corporporate people

    Michiana Health and Rehab is a huge a$$ joke. The administrator and d.o.n DO NOT care about their residents nor their emoplyees. Everybody is over work and not appriciated one bit. If you are not a part of their "click" then ur screwed and will get screwed over. They have lost several good employees since the admin has been there and the building has gone down hill quicker then you can blink. I wouldn't recommend my worse enemy to that place.

    Response: The staff can try all it wants to provide proper care but when the corporate dimwits come in and screw everything up... well, it becomes impossible. The DON and Administrator, two of the worst in their field that I have seen so far, don't give a @#$% about the staff or the residents. "Come in and tour" sounds like this review was written by the equally idiotic marketiing person.

    Response: It is too bad that all the bitter ex-employees can just post whatever they want rather than having actual people who have experienced a stay in the facility say how the experience actually was.

    all of our staff try our best to provide appropriate and timly care to our residents, The staff strive to ensure saftey of our residents as well, The most important key behind success are nurses that are nurses because they truly like being a nurse. at this facility i feel that we have exceptional staff, things have changed from the past, just come for a tour, you will see.

    I was very disappointed in the way the building has been managed by the Administrator and the DON. As I found out , the administrator doesn't have a care for the patients or family. My son had gone up to the front desk to ask to talk with her, she told the ADON at the time come talk to him,about "what ever"!( He overheard her). The Extendicare company needs to really look at their past record in other facilities. I'm having another surgery soon. Looking at other avenues.The Don by the way does not give any eye contact.!

    Response: The time span on most of these posts is about the time the facility was going through major changes. And that is not what this site was set up for, disgruntled employee's that obviously have no business caring for people.

    I would NEVER recommend putting your loved one in this facility. Yes, it is a beautiful building, but don't let that trick you. Administration is horrible and I believe cares very little about their residents, or their employees, and are more interested in collecting that $$$ each month. Staff is constantly short, not allowing the time or care that the residents deserve. I left the facility voluntarily because I refuse to work somewhere where the residents are not receiving the very best possible care. Staff is constantly being hired, fired, or quitting and is over-worked. The staff that does remain, very seldom stays on one specific unit. RN's, LPN's and CNA's are being rotated daily, which confuses the residents and does not allow for the caregivers to become familiar with them, their needs, routines and medical conditions, which is very important in carrying out their individual care plans.

    Nice new building & pretty interior, but the things that matter are lacking. Administration does a lousy job sharing information and providing a care plan with immediate family. I had to ask for a meeting to get it. YOU must be the pro-active one or you won't get much information. I told staff several times that my Mom was trying to get out of her wheelchair & bed unassisted, but it was only when she she slid off the bed onto the floor that they put an alarm on. One on the chair took longer.
    The care staff are pleasant and helpful, but were obviously uninformed about her condition and needs. Several times staff came in to give her meds that were canceled within the previous week.
    My Mom came in with a broken hip yet the care staff kept asking me questions. Her file should have had the answers. Don't know if they didn't read her file before attempting to provide care or if the information wasn't available in the first place.
    Rehab was minimal and no one walked her around other than in the rehab room itself once a day. She started getting stronger then declined.
    She is no longer there.

    This facility is beautiful, but don't let that fool you like it did us. There have been several mistakes made with my mom who suffered a stroke. The final straw was this week when a medical test was ordered and we were waiting for the results, only to find out that tour Dr.'s order was never carried though. My mom ended up in the emergency room with a severe infection because we were awaiting the results from a test that was never submitted. Also, she is on a pureed diet and on one day they brought her a regular sandwich. It has been one thing after another there and we've heard that leadership has a revolving door...unfortunate for such a nice new facility, but more unfortunate for my mom who is suffering more due to their negligence



    Response: It is worse today than ever. So sad to see our loved ones in this place. The nurses and aids work so hard but they do not hire enough help more profit that is all they care about and I bet they pay the people poorly.
