Maple Grove Adult Foster Care is an assisted living facility. Assisted living facilities are an apartment-style habitat designed to focus on providing assistance with daily living activities. In addition to the above program elements, it is my intent to utilize local community resources including the library, and shopping centers, we also have a huge deck, with a handicap ramp and cushioned seats. These resources provide an environment to enhance the quality of life and increase the independence of each resident. My fee is $1200.00 monthly and I do accept some state aide.
Michigan Department of Human Services, Rule 400.1407 (9) states that the resident or the resident's designated representative provide a written health care appraisal completed within 90 days before admission to the home, or not later than 30 days after admission. A department form shall be used-BCAL 3947, I have those forms available. I also use Hospice of Michigan services when necessary, very professional, caring and compassionate.
