My Mom was at Park River Estates for 15 days while she recovered from surgery. I was hesitant at first because of what you read in the paper, however, they change my mind. Thank you to all of the staff that took care of my Mom! If she needs care again, she WILL be back!!

    This care center is very unprofessional. While I was visiting a family member there I heard one of the staff members talking about sex and she smelled like marijuana. (Stacy N.) was her name. Our nurse was not even the greatest. My family member left there worse then she already was. I wouldn't recommend this facility to my worse enemy. I have reported them to the state.

    Park River Estates Care Center is an average sized, for profit, nursing home with 99 beds based in Coon Rapids, MN. At last check, the facility had 90 residents indicating that it is 91% occupied which is about average within the state of Minnesota. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
    This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Park River Estates Care Center, was 4 stars, which is a better rating than 82% of nursing homes in Minnesota. Important to note that this facility has recently detriorated and has gone from 5 stars to 4 stars.
    We have compared the detailed Medicare data for Park River Estates Care Center with other senior care providers in Minnesota. When compare to the state averages for staffing, the number of registered nurse (RN) hours per resident per day is about average; Certified nursing assistant (CNA) hours per resident per day is about average; The number of licensed practical (LPN) or vocational nurse hours per resident per day is about average.     I was a patient in the rehabilitation unit because I had a knee replacement. I am so happy how everything turned out that I told the staff that when I get my other knee replaced, I'm coming back. Thank you Park River Estates staff & therapy department!

    I give this facility 2 thumbs down! While I was there visiting, I was informed by 1 of the staff members that she was working as an LPN and was never licensed!!! And to top it off, my family member was discharged with 2 infections that had been present for several months without treatment, according to lab results. They were also overcharging medicare for services that weren't needed.


    Response: I have been the administrator here at Park River Estates Care Center for 12 years and in my profession for 30 years. The one thing I hear many times from family members and residents is that they can approach us with any concerns and that they are very thankful for the care we provide. Unfortunately, the person that wrote the comment on 12/17/10, did not let us know what they heard or experienced. This person could have made a complaint to the MN Dept of Health. If the complaint was made, we would have known about it during the annual survey by the department of health which was held from 1/10/11 - 1/13/11. The result was one D level citation.
    With respect to "a person working as an LPN and not licensed", we have policies in place to verify with the MN Board of Nursing prior to hiring an employee to see if they have a current license. After hire, a nurse has their license renewed every two years. We again verify with the MN Board of Nursing if it had been renewed. If the license expires, the nurse is removed from the schedule until a valid license is verified. We also perform two background checks to make sure we are hiring a qualified person and not a criminal.
    The care of each patient here is under the direction of a licensed physician. All treatments are based upon results of various tests. We have two physicians that specialize in geritrics, have been here for over 15 years and are the attending physician of 98% of our patients. They are always willing to discuss concerns that a family member has.
    Payment by Medicare is based upon a level of care and is all-inclusive. That payment is fixed. If for example, the Medicare level of care is paid at a flat $300/day, we can "charge" Medicare $300, $400, or even $500 per day and our payment will still only be $300/day.
    Finally, we have had many employees have a relative come to Park River Estates Care Center for either short term or long term care. One specific employee had his mother here for care on two different occasions. She was even a Registered Nurse at North Memorial Hospital for over 32 years. She was very impressed with the caring employees and how long employees have worked here. Many employees did not even know that the patient being cared for was a relative of an employee. That employee was me, Tom Pollock, Administrator.
    I encourage everyone to visit us and other nursing homes to compare. As the information stated above, we have received 5 out of 5 stars by the Medicare.gov website and are the only 5 star in Anoka County.
