At last check, the facility had 105 residents indicating that it is 100% occupied which is about average within the state of Minnesota. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.

Poorstaff and administration. Unresponsive to my mother's needs.She had extensive bedsores and we found out from the UNDERTAKER? No communication with family. They keep crowing about their wonderful building, not its rating which is 2 yes 2. Pathetic.

The Pioneer Care center on Mabelle Ave. in Fergus Falls, MN has been a lifesaver for our mother as she deals with getting over bronchitis so she can go ahead with lasertripsy for kidney stones. The staff is always happy to see us and all are willing to relay messages to doctors if there are questions or changes needed. We are so thankful for the existence of this new facility. It is beautifully decorated and provides so many services that combine the best of home care and community inter-actions. Food is carefully planned and personally served in the dining room which creates a family-like but organized mealtime. What a boon to the region!
