Very sad and worn out facility. High nursing turnover. Rude nurse. Former social services rep has terrible family relation skills and is now asst. administrater. Smells horrible - especially on the weekend. Residents left in hall. Only decent staff seems to be in activities.

I took my mom out of this facility. Very poor care for Medicaid residents. Poor communication between staff. They need to train their lower level staff that care for the residents. Resident rooms need maintenance. Poor medical attention. This was a very bad experience for our family. I would not recommend this facility to anyone.

Adminstration, social servics, nursing staff are horrible. It always stinks. Dining room chairs are falling apart. semiprivate rooms are cramped. One nurse in particular is a very gruff and unhaapy person. Staff yells at residents.

This is one of the worst nursing facilities I have ever been in. Staffing is low, staff audibly bickers among themselves and they quality of care is extreamly lacking. No updates have been made to the facility in years- very dated and in need of some serious work. The dinning room roof was supported by 2X4 braces last time I was there.

The facility smells, the rooms are unkept, one of the nurses is especially rude to the residents as well as the visitors.
