Evergreen Mountain View Health & Rehabilitation Center is an average sized, for profit, nursing home with 146 beds based in Carson City, NV. At last check, the facility had 135 residents indicating that it is 92% occupied which is about average within the state of Nevada. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Evergreen Mountain View Health & Rehabilitation Center, was 4 stars, which is a better rating than 85% of nursing homes in Nevada. Important to note that this facility has recently improved and has gone from 3 stars to 4 stars.

Response: how anyone could put there loved one here is beyond me. First of all try calling the state bureau that inspects this facility. It is way below Grade - like an F-. They threatened to shut them down. If it looks nice on the inside; it says nothing about the care and how they treat the human beings inside. Go in and check for your self at random times. If people are in there asking for help and moaning and groaning, ask them how long they have been waiting for help since they pressed that buzzer.

Response: I worked there as an RN, I know what she is saying to be true. In fact, it is way worse than what she says. Many patients died there. The corporation that owns these Evergreen facilities are greedy and corrupt. Very top heavy, when they need to spend the money on staff! All Evergreen facilities need to just go away!

I recently visited this facility as we had a friend of the family there. The facility was clean, the staff were friendly, they went above and beyond to make us comfortable and provide all the amenities that our friend needed.

Response: You are wrong. I worked there as an RN. I know how bad it was. Deplorable conditions, poor management, worse than poor treatment of the residents. Many patients died there in the past. It is one of the lowest Medicare rated facilities I've ever seen. It was a horrible nightmare just working there. I felt like a cat on a sinking ship, watching the rats abandoning the patients like they would a sinking ship.

To anyone thinking about placing a loved one in Evergreen Mountain View Health Care facility....DON'T DO IT. These people are horrible care givers. If you ask for a shave, two days later they'll say O, I'm sure somone will do it today. The sheets are dirty and urine standed, and you will frequently find the help buzzer on the floor unexcessable. They treat able minded and alert patients like babies. Two of our family members are in this facility--great people whom we love dearly. People who have worked hard all there lives and given soooo much to the community and their families. How DARE you Mountain View...treat our loved ones with such disrespect and indeciency. If you work at Mountain View, ask your self: If this was my mother or father, what kind of care would I exstend? If it is anything less than the very BEST, get a different job. SHAME ON YOU PEOPLE!

Response: It's too bad you had a bad experience there. Did you ever speak to the management? Did you move your parents to a different location? I had a loved one there and I had a great experience, and if I ever had any problems I spoke to the Director of Nursing or the administrator and they always took care of it for me, so if you loved ones are still there, I would talk to someone about it.

Response: I was an RN there! You are dead wrong. It is actually much much worse than what she said. I don't see how you could say it was good. It was deplorable conditions, staff are lazy, they steal. Oh, it's way way worse. I wish I had the time to write what I know about that place. You would be sickened.

Response: PLEASE email me at [email protected] I just moved my mother out that horrid place. It would be helpful to speak to someone that worked there. I get the feeling that she was mistreated, she acts odd when I ask if anything happened that I need to know about.
