o We establish a core team of stable therapists, rather than inconsistent per diem staff, to meet the varied needs of our subacute and long term residents. New admissions are greeted upon arrival to the facility, provided with appropriate equipment, and evaluated in a timely manner. The rehab department is dedicated to working toward effective communication and optimal teamwork with all members of the interdisciplinary team at your facility.
• Unity of Rehab / Nursing during Admission Process:
o Upon nursing addressing the immediate care and stabilizing the medical status of the patient, the rehab team is ready, willing, and able to provide therapy. We provide treatment at bedside, with a strong emphasis on caregiver education in the first critical hours. All disciplines work under an individual scope of practice with clear communications toward a unified goal of positive outcomes for our residents. Our cohesive approach to patient care reduces the potential for hospital readmission. As the patient continues to progress, we modify the plan of care consistently to most appropriately meet patient-centered goals.
• Discharge Planning:
o Within the first two weeks of admission, rehab is represented in a care plan meeting to discuss the plan of care and goals for the client. The therapy department will provide tools and strategies, such as individualized home exercise program and durable medical equipment, for a smooth transition to home. Long term residents are provided with appropriate restorative nursing programs and monitored together with the nursing. Continuous specialized education is reinforced from initial evaluation through the discharge process, including, but not limited to: energy conservation techniques, joint protection strategies, adaptive equipment training, gait and transfer analysis, cognitive retraining and safe swallowing strategies.
• Marketing / Education
o Education programs are the best tool for marketing your facility. Prime Rehabilitation continuously provides presentations in your facility to therapists, nursing departments and outside health providers to assist in marketing efforts to build facility census.
• PPS Knowledge and Expertise
o We provide supervision of RUG utilization, MDS questions, double-checking / correcting the monthly billing, advancement with changing regulations, and documentation. On-going education is provided to team members to keep up to date with all the numerous regulatory changes. Much time was invested for the transition to ICD-10; PRS educates both the Rehab staff and facility personnel in selecting correct codes for adequate compensation.
