I am writing this review for one of my patients. I work at Avalon Gardens and like to chat with my patients to see how they are doing, if they have any complaints or issues that need to be dealt with. When I spoke with Bill, he stated that he is very satisfied the way housekeeping is keeping his room clean. He said that his housekeeper Rogie was great and thanked me for asking him questions about his room's condition.

    One year ago tonight my husband spoke to me for the last time, asking me not to leave until he was put safely into his bed. He had been begging me to get him out of this rehab, but the physical therapy team would not release him until he could walk a certain amount of feet. He complained to me and to management that one of the aids had sexually abused him. The aid told me that she told management that this was not true, and management believed her. The situation was that the opening in the bathroom was too narrow for the wheelchair- and so he had to use a diaper- and this is when, as he told me, he was abused. The air conditioning was put on so high the man in the bed and my husband (as well as myself) were shivering. Residents were roaming the halls selling things. Residents were threatening patients. All different types of residents were put together. Two days before, he was found unresponsive, I tasted his dinner after arriving at the home. The food tasted so bad that I had to spit it out. The night after, my husband complained to the male nurse on duty that he was having bad stomach pains. The nurse spoke harshly to him, asking what he could he do about it. He then told my husband he would let the doctor that came in the morning that he (my husband was not feeling well) My husband begging me to make sure not to leave until he was put in the bed. I told the head nurse and had to leave. The next morning he was found unresponsive with septis, and 93 temperature. He was taken off of life support at a nearby hospital on September 30 and his heart stopped October 4, 2016.

    Avalon Gardens Health Care Center is a great facility. Of course their nursing care is great and their therapy services help me a lot. But I especially like the social aspect. I get to play Bingo. I also spend time socializing with staff and other residents. I find everyone to be very friendly and respectful. I am not that crazy about the food. I think it could be improved. Other than that, Avalon pays attention to me. They care about what I need and solve any problems that come up. I also feel safe which is very important. I really recommend Avalon

    Recently I was a patient at Avalon Rehabilitation Center. The nursing care I received was very satisfactory. The staff were quite friendly and the nurses and aides were always pleasant and helpful. My therapy was just what I needed and they paid attention to my needs... including any problems that came up. The overall safety at Avalon Gardens is good, as is the laundry service. Unfortunately, the food was not exactly to my liking. Overall, I got the professional care I needed and that was most important to me.

    I am at Avalon Gardens home due to a stroke. I find the nursing care and therapy services to be excellent and very professional. I like doing many of the activities that they provide. Avalon always attends to my individual needs and responds to my problems. Overall I am very pleased with the quality of care and attention that I am receiving.

    While at Avalon Gardens Rehabilitation and Health Care Center for a leg condition, I got to know the staff. They are wonderful - extremely nice and treat me with dignity and respect. The social workers always have taken care of all my needs. The nursing staff are caring and professional. I feel that people try their best to cater to my individual needs and respond to any problems.

    IT SHOULD BE SHUT DOWN. PATIENTS ARE IGNORED. THOSE EMPLOYED DO NOT FOLLOW INSTRUCTIONS. PATIENTS ABUSE OTHER PATIENTS AS DO EMPLOYEES ABUSE PATIENTS. NO SOAP OR SANITIZERS IN BATHROOMS. NO LOCKS ON ANY BATHROOMS. OUTSIDE WELL LANDSCAPED, INSIDE IS FALLING APART


    Response: Not sure where you had gotten your information, the home is well maintained and even going a full renovation as I write this post. The patients and givers behave very well, although it is near impossible to handle all patients behavior, the staff is well trained and treats everyone with respect, patience and provides the best care given each patient's circumstances.

    Response: That is the smellest place ever...The director of nursing should be fired.

    Response: It is the smelliest place ever.The staff failed to let anyone know that my mother passed at away at the nearby hospital,they are very rude when I got there today Mothers Day to surprise my mom,they just told me in the hallway my mother is dead in front of other people and staff showing no compassion at all.Then they lied to me saying that they did contact someone to let th know.Neither me or my brother were notified!My mom rest in peace, was crammed in a small room sharing this tiny room with two other people.It's a dump and very unprofessional l

    my father was sent to Avalon Gardens for rehab after a fall and within one week his health deteriorated to the point that he was diapered and babbling. He fell twice and wound up in the hospital with dehydration. It is a hell hole.

    Avalon Gardens Rehabilitation & Health Care Center is a large, for profit, nursing home with 353 beds based in Smithtown, NY.     a discusting facility!!! My sister was there for 4 years. Meds were not properly given, reoccurring urinary tract infections, filthy room, especially sink area. Finally got her out and she is in a much better place. The day she left felt like I saved her from being in a war!!!!!!!!!!!! Never, ever send anyone here!!!!!

    Response: Disagree. I was there daily as a caregiver for over two years. The staff is hard-working and there was NO physical abuse.

    dont ever send anyone there. it is disgusting.it smells like urine.it is bed bug infested. they have crank up beds. there are no telephones, you have to have verizon come and set it up yourself. they forced my father into a diapeand were so vulture like.r and thankfully we got him out of there. i can't believe that they are allowed to stay open/all they care about it making money.they should be shut down

    My friend has been a resident at Avalon Gardens since April 22, 2010. He has endured long waits (sometimes over an hour) for responses to his light/bell when he needs assistance. He was recently found to have been over medicated, but that was corrected. He seems to get the best care during the day on weekends, and the worst is in the evening and overnight. He has been subjected to sleeplessness due to excessive noise in the evening, and is sometimes awakened at 4:30 AM, very abruptly, to take blood samples for lab tests. Though he is to receive wound care for pressure sores, the quality of that care varies from adequate to poor.
    Yesterday, after I lodged complaints about this treatment, rather than address ANY of the complaints, I was told his discharge from the facility would be expedited, even though both he and I feel he is in need of further care.
    As his health care proxy, I find this facility severely lacking in overall compassion for the patients needs.


    Response: Avalon Gardens is a disgusting place. I've witnessed a lot of abuse there first hand. The head social worker Rosemary Pititto steals from the residents. She has stolen a cross and chain and over $200. etc. from residents. She steals their valuables in front of them sometimes. The place smells like feces 24/7. The aides if paid bring in liquor, cigarette, etc. for residents. Some aides have sex with residents. When they get caught, they take it out on the resident. Some are kicked out and others are sent to mental hospitals to discredit them. Avalon Gardens should be shut down.
