08/02/2015 An Interested Visitor Writes: This place is bogus. Had a brief stint there earlier this year (in March) and they are disorganized. They hire anything, they didn't have real social workers at the time and probably still don't. Real social workers with degrees don't stay.

The admissions "social worker" is really a CNA, no formal training and the discharge "social worker" was actually a chain smoking housekeeper, no formal training in social work. Can you believe that?

What an insult to all BSWs and MSWs who went to school. This place is unprofessional to hire anything off the street and have the nerve to give them business cards with their names and the words "social worker" on them.

06/08/2015 An Interested Visitor Writes: From December 2014 through April 2015, our mother was at Lake Park Nursing and Rehab for short term rehab after falling. Having had no experience with nursing/rehab facilities in Charlotte, we were uncertain of where to send mom after her hospital stay. We asked hospital staff and others what they knew of LP. They all gave it a good review. One of the EMTs transporting mom stated to us that LP might not be the newest rehab in town, but the staff made all the difference. And, the EMT was correct! I truly do not understand how LP could receive such negative reviews that I've read on this site. Our mother received excellent care while at LP. The PT/OT staff were kind, caring, compassionate, and knowledgeable; they were always brainstorming to come up with solutions to help my mother get better and stronger. If something didn't work for our mom, they tried something else until they found a solution that worked. The CNA's were very kind, caring, compassionate, friendly and engaging! They always made our mom laugh. Shaneethia, Kiley, Brittany...all excellent CNA's! Joy, Jackie, Jaimie, and Roslyn were exemplary nurses. Anytime something changed with our mom, we received a call from Nursing informing us of the change, whether it was her medication or a condition, we always received a call. Kim, the Administrator, was always accessible and was very professional, kind, compassionate and caring. Sharon in Accounting was extremely knowledgeable with regard to insurance and Medicare, and if we had a question, she would give us, or get us, the answer...all of the staff went above and beyond to meet my mother's needs, answer our questions, explain things to mom and us, and provide the best care. And by the way, one could call any time of the day or night, and someone answered the phone and routed the call. As well, if they said they would return a call, they were true to their word and returned your phone call in a timely manner. Ray, Adrian, Barbara and Luna in PT provided excellent care and encouragement. Our mother was so much stronger after leaving LP than before she arrived! I know I am leaving some people out, and I apologize for it, but truly, our mother received excellent care, and we would not hesitate to recommend LP to others.

    05/11/2015 An Interested Visitor Writes: My mother has been at the facility for quite some time and still seems to not be getting the care that she deserves. There has been new management put in place, and facility still seems to be deteriorating (if not getting worse than it was because of the new management). I am not sure I can keep my mother here for much longer and watch her health desinegrate from, of course, natural causes, but also because of the facility. Please do your homework before sending your loved one there.

    01/07/2015 An Interested Visitor Writes: Nothing !!!!!!!!!!!!!! If you care about your loved ones Do not put them in Lake Park......the place is horrible....from Management ( if you can call it that ) all the way down. If the State Health Dept. had regulations on Nursing facilities (which they don't) this place WOULD be closed. Trust me don't make this your Choice !!!!

    09/27/2014 An Interested Visitor Writes: The place stinks. This for profit company have built a new building in Mint Hill, NC while cutting employees benefits. Each time there is a heavy rain fall the building floods and stinks to the high heaven. Census is at a all time low. so are wages resident care and employee morale . They have promoted a director of nursing who does not respond well to family complaints, she rolls her eyes when she see's them approaching her, moreover she marches around like a menacing bulldog. This is the south not Brooklyn New York. They should send her to seminars to improve her people skills if not , her reign will be as short as her temperament. I pray that this company clean up their building restore benefits with a decent pay raise and promote competent people. Until such time the building will stink.

    06/26/2014 An Interested Visitor Writes: Lake Park Nursing & Rehabilitation Center doesn't even deserve a one star. This place keeps it either so hot, or too cold. The elderly rides around in their wheel chairs with ear muffs on, big coats on. I tried to get them to warm it up for my mother, and everyone said they could not touch the thermostat. Well, someone there had a key after all, however, not for all of them. Only after I got upset and threatened to take my mother out, is when they found the one lady. Then called the maintenance man to come in and adjust them.. They truly don't care about how the elderly is cared for. My heart and soul could not work their daily and see what goes on. Some nurses actually care, and clean up the beds and do as their duty requires. Others never changes the sheets, just dabbles in there and leaves. Not doing their job. So, if the staff of the help who actually tends to the patients are not doing their jobs, that means the people who run it, manage it, and own it isn't doing their jobs. I would never ever place anyone in this facility! This place stinks, and the bathroom has a terrible aroma, they NEVER CLEAN INSIDE THE TOILET! ONLY POUR SOLUTION AND FLUSH IT!!! ITS HORRIFIC ODOR!!! I could say a lot more that is horrific, however, now you know the certain little wrongness this place provides! You've been warned! DO NOT USE LAKE PARK NURSING & REHAB CENTER, in Indian Trail, NC!

    lowest paid.. The turnover in CNA's is unbelievable....The patient's suffer the most...
    Don't send your loved there unless there is no other place.

    is unbelievable....and they totally run the place.
    Lowest paid but do ALL the work. Think twice before
    sending anyone there.

    08/04/2012 An Family Member of Current/Past Resident Writes: My mother was admiited to Lake Park for rehab for a stroke. After weeks of therapy she was in worse shape than she was when she first entered. Luckily my wifes background as an EMT afforded her the knowledge to address certain problems that may have gone unnoticed and we got her out of Lake Park. The social worker who is supposed to be an advocate between the resident and facility was never present during my mothers stay. However I did get to speek to the assistant social worker three different times, the first during a family care plan meeting and when concerns were brought up she stated she would schedule another meeting in 1 or 2 weeks. After 3 1/2 weeks and no meeting I called her expressed more concerns, one being the lack of proper care my mother was receiving she replied with "we're not perfect", I guess if it were her parent/s in the facility "thing's would be perfect". With this all being said, the director of nursing, administration staff,nursing staff, and and social services staff are all a joke that don't even follow the policies they are suipposed to. The only ones who truly deserve credit are the CNA's and unless told opt not to check for or change "dirty" briefs unless told.

    05/03/2012 An Interested Visitor Writes: I have read these reviews and I have had my Mother at Lake Park twice in the last two years for therapy. My Mom loved it there and felt that she was taken good care of. We had a problem while there and the Director of the Nursing handled the problem with speed and we felt good about how she handled it. I was on this site to show a cousin how to look up nursing homes for her mother. Of course I pulled up where my Mom had been to show her and here is all of this negativity. That was embarrasing after I told all the wonderful stories of Mom. It is obvious that someone doesnt like the Director there but they shouldnt abuse this review area by blasting personnel concerns when the potential customers are who review this. No offense to the person posting, but family members dont care about C and As problems with administration, they care that Mom or Dad is being cared for by those C and As.

    Response: It's great your loved one received such wonderful care. I however had to remove my mother from Lake Park because even after she reported falling, my wife and I are the ones who had to take her to the ER because the nursing staff felt her change in demeanor was from a UTI and even when that test came back negative they still opted to do nothing. Luckily we got her to the ER when we did, because who knows what physical defecits she could have and may still have suffered from her cerebral damage. And FYI the CNA's (certified nursing assistants) are the ones who do the most in taking care of residents while the nurses hand out med's. This review area is for just that, reviews and soimeone decides to air their "dirty laundry" about a certain place, area, or whatever. They have the right to. It's called freedom of speech, look it up.
