Two double rooms (4 people) expected to share one toilet! Tiny rooms, staff never smiling or happy, have to ask for a locked up folding metal chair to sit in if you want to visit someone. The food is terrible. I had to change my mother-in-law's bed myself after I asked for clean sheets.

My grandmother was here for 20 days post fall and needed rehab. Earlier this summer she was in another facility in Vandalia. This other facility is big and beautiful and can be seen from I-70 and I-75. It even had a grand piano on the second floor. Let me tell you. This facility has only its looks going for it. There were so many negative things about this other facility that Spring Creek had little to do to improve upon care. Thankfully, they improved by leaps and bounds. My mother left there in better shape than before her accident that took her to the other facility. The staff were caring. The facility was clean even for an older building. Any issues were met with the due diligence of the staff and resolved immediately. I will definitely recommend this facility to others.

Patient punished by staff all day, forced to sit in a wheelchair for hours by the nursing station. Meanwhile the patient has been out of it all day. No one on the staff notices the change in the patient's mental status. Family had to request that the physician be called. Physician paged eight times and never called back. Family has to request medical transport to ER. Patient totally incoherent and also dehydrated. How would any member of the staff feel if this was their family member. Don't take your family member to this facility.

Response: Patient was having a stroke and no one here has the education, ability, or compassion to do anything about it. Good thing the family stepped in and took control of the situation.

My 83 y/o grandmother needed rehab after a total hip replacement. We had to choose a facility last minute and chose Spring Creek based on Medicare ratings....that was our first horrible mistake. Spring Creek did not inform their staff that we were arriving even though they told us to come. They tried to put her in a double when we requested and confirmed a single room. The whole place was incredibly depressing with patients just sprawled in their beds unaware of what was around them. The "double" rooms were actually singles that they had squeezed an extra bed into to be able to take in more patients. They finally put her in a single room, but she had to share a bathroom with a male patient. We kept complaining about the deterioration of her mental state (she was previously sharp as a tack, no mental deficits), and they told us "sometimes this happens and you have to prepare yourselves"...Turns out they were overdosing her on muscle relaxers to keep her compliant and unable to perform well for her rehab sessions so she would have to stay longer. When we tried to take her home because we were supremely unhappy with the facility, they told her that if she left against their medical advice, her insurance would not pay for any of her stay! She went in voluntarily and her insurance company told us that they would never refuse paying. The doctor of SpringCreek could never be reached, even when her personal doctor tried to contact him. She went almost 48 hrs after arriving before they administered her first dose of medications, and couldn't tell us why there was a delay, nor would they let us bring her meds from home (she has a heart condition). Staff were incredibly loud all night long and sleep was nigh impossible. Breakfast would come at 6am, meds at 9am, lunch at 11am, dinner at 4pm, meds at 12am, the lab tech would come in at 3am for blood draws, etc etc. All of these times are approx because, as we were told, "there isn't really a schedule". We didn't know from day to day when she was having therapy, meals, or meds. The therapists continually told grandma to do motions that were against the precautions her surgeon had given her. Not to mention these actions caused her to pull her groin muscle which set back her recovery. The therapists demonstrating the exercises didn't follow proper form and didn't require pts to either. All in all a horrible place! We've filed grievances with the state, the county, and her insurance company. Please do not send your family there!

Response: The staff at this facility is very incompetent.

I visited a friend on Fri. April 1 st and 2nd. The smell was so bad, we could not stay. The whole wing to the left of the front entrace smelled like pure @#$%, my 44 yr old son and his wife were gagging. How can people work there and not smell it? Someone is not doing their job. I can't wait to get my friend out of there , and I will send a letter to the state about this. It's ridiculous
