My loved one literally starved to death at Jackson's 4-Winds. She was a "help-feed" but repeatedly, I found her trying to feed herself. She was a "swallow-hazard" but was left alone in her room often at mealtime, a tray placed in front of her, and water across the room near the sink. Though bedfast, she was a "fall hazard". Despite this, she had many falls from her bed or from an ill-fitting wheelchair. She was 6 feet tall and in an ordinary chair. Her last fall sent her to the hospital to receive 8 stitches in her forehead. Witnesses said she lay in the floor for 30 min. before being transported. Her local POA was not notified, nor was the secondary person listed (me.) On the day she died, she was denied any pain meds, her doctor was not called, nor was Hospice. She suffered a long, difficult day without any relief from the pain she most certainly experienced. It was a gruesome passing .
