This facility has had it's ups and downs and right now is truly a down! The rooms are very dirty, the staff that is in charge of keeping them clean does not do a good job. I am not sure if they are understaffed or do not care. It has changed administrators several times. The last two Admins were great but the staff does not like to work hard so they ran the admins and DOR off. Management at this time are two nurses that do not understand their jobs and do not care enough about the patients. There are some very good nurses and aides in this facility, however, they cannot do all the work. My family member is there for lack of a better and closer choice. The activities are numerous which is a plus. The ladies that run the activities do a great job. My family member has stated the food is fairly good and she generally eats her meals. She has had to sit in her own urine and feces and food all over her several times. We visit on almost a daily basis to keep her clean and make sure she is taken care of. I would not recommend going here long term. The therapy staff is awesome. I think they have the best rehab there is. If I could give a second rating, I would give them a 5+ star rating. They do a great job and care about the patient and the progress they make. Too bad the facility is no the same.

    My mom was taking to concord for her final days. I could not have made it without the staff and management who helped me with her care and the transition of having to use long term care. I want to give a special thanks to Chris the DON who took the time to visit with me and explain the process that was about to happen. The nursing assistance and nurse were awesome...thanks guys for everything you have done. There's a special place in heaven for you guys.

    My parent died from sepsis due to a bed sore that measured 16 inches in diameter that was not properly treated while residing at this facility. While on site, I observed vent patients regularly waited 30+ minutes for attention while their monitors where blaring and their vitals unstable. It appears to be the best place to ensure a hasty demise.

    I would not recommend this nursing home to anyone, they do not have enough staff to cover the patients an the ones who needs care they do not even acknowledge them. They leave them in bed an at night time they do not put their night clothes on them they leave whatever they have on that day to sleep in. They prescribe them meds to sleep so they don't have to deal with them. Then after their death they don't acknowledge the family with any remorse, not an I'm sorry, flowers a card NOTHING!!

    Response: We'll take care of your family member instead of dumping them at a nursing home. If it's sounds simple try it.

    Many staff do not appear to be properly trained in care of the elderly. Meals are not geared toward residents' special dietary needs. Administrators do nothing when residents' personal items are stolen, broken or damaged by nursing home staff.

    My mother went to Concord after being in KDMC,St. Mary's, and Cornerstone. She went to Concord because she was on a ventilator. Jason, the respiratory therapist was able to get her off the ventilator in about 3 weeks. We were told at KDMC that it would take 6 months to two years, if ever. I want to say a special thanks to all the people who took great care of my mom. Julz, Julie, Debbie, April, Eric, Rebecca, Gary and Britainy. I also want to thank the physical therapists who worked tirelessly to help my mom regain her strength. Monique and Charlene worked with mom everyday. Steve and Joe thanks for giving her 2 extra reasons for working hard to get away from you. Thanks to Kelli for getting mom her first taste of food in months. Ths staff at Concord is the best in my opinion.
