Putting my mother here for one month is something I will regret for the rest of my life, the place was a disgusting mess, everything was old and run down, (which is surprising considering the area it is located in), it reeked the minute you walked in the door and the food was nasty looking, there were never enough people working to take care of needs in a timely manner, call lights buzzed continuously while the employees laughed and joked in the hallways. Poorly managed. The state has to force the owner to make repairs or things don't get fixed. If you love your parents keep them away from anything this man owns, he's the kind of person you end up wishing you had never met.


Response: Very sad news, the facility used to be good when I had my loved one there back in 2011. The administrator was very kind and was making many improvements. Sad that the residents are the ones who suffer.

This place is a dump. Shame on the owners of this place for making people live under these conditions.


Response: The state had to force them to clean it up, but at least they are finally doing something about it. They were going to lose their funding if they didn't clean it. Says something about the owners.

The administrator is very concerned and attentive to the needs of all the wonderful residents we work for. It is a nice, friendly, and very caring staff also. We also have a great activity director, that engages and keeps the residents very happy.

    Response: Care is so great?, so that's why a resident fell and fractured her femur. They got cited on this by state on 10/21/14 complaint investigation.

    State finally cited them for having an unclean smelly facility. Hopefully things will change. Very degrading to the residents who live there.


    Response: It is about time! It was filthy and smelled terrible when we were there in 2010. Shame on the owners for letting it get so bad and not caring about the condition of this place while they built a new multi-million dollar facility in a nearby town.

    The Cottage Extended Care is an average sized, for profit, nursing home with 66 beds based in Tulsa, OK. At last check, the facility had 59 residents indicating that it is 89% occupied which is more than average within the state of Oklahoma. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
