Dangerous environment for patients in need of skilled nursing care and/or rehabilitation. Why does the state of Pennsylvania allow them to operate?
Shut them down immediately!!!

I was injured by a nurse on my first weekend there. Some nurses are good, some don't care. One nurse continually gave me a different med than what was on my chart. Sometimes they just don't give you your meds at all. If you hit the buzzer for a nurse, sometimes they come quickly, and sometimes they don't come at all. They have no business taking in the younger rehab patient, as they are only set up for taking in the elderly dementia patient, who can't tell what meds they're getting, can't tell there's no cable TV, can't tell there's no internet, etc.
