Americare Health and Rehabilitation Center is a small, not for profit, nursing home with 46 beds based in Memphis, TN. At last check, the facility had 134 residents indicating that it is 291% occupied which is more than average within the state of Tennessee. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Americare Health And Rehabilitation Center, was 2 stars, which is a lower rating than 58% of nursing homes in Tennessee.

My relative has been in this facility for a couple of years. When I started going there to visit in Sept. of 2010, a foul odor would hit me at the front door. The odor is not as loud anymore. A broom is kept in the room so that we can sweep the floor and couple of times we have taken towels and washed the floor. The nurse spills feeding in the floor and leaves it there. Roaches are bad in the room. My relative never leaves the bed unless we ask. My relative is allergic to pampers yet they continue to use them. The nurses act as if they can not help with manual care (patient care) and the CNA's are rude and always on a smoke break. My family has been told we can not move unless we go to the hospital but is not granted a visit to the hospital. I have seen bruises on my relative, complained about them, and it is as if I am talking to the wall. I can not give this place a 1 rating this place is very upsetting to my family.

In seeing this facility. I can classify it as a "warehousing of longterm care patients." I did not see any quality of life events (i.e., excercise programs, events, religious outings, etc...) Most of the residents just watched television and/or roamed the halls. Patient rooms smelled of urine and the facility was overran with roaches. Overall, this facility is ill-kepts and in need of upgrading/updating.

Response: I concur with the above assessment. Americare is poorly run and the staff is noncaring and uncooperative. The nurses have no control of the patient care staff and the staff does not properly care for the patients of the facility. A calendar is posted in the rooms of the patients of acitivities but none of the these activiities occur. They only clean up and pretend to care when the state is inspecting the facility. The administration/owners do not respond to complaints of possible abuse/neglect of patients and appear to be only concerned about the money. This is a horrible place to spend your last days of life and we are trying to get my father out of this place but the administration has not cooperated with the completion of the paperwork. Every time I visit my father I end up helping other patients to their bed, getting dressed or to the bathroom, because the staff chooses to ignore their requests for help. When you ring for assistance they say okay and but did not come in the 2 hours I was there. When I left they were all just sitting around at the nurses station. The staff complain about being understaffed but I don't see any of them working too hard. The patients are not assisted with eating or even given ice water when they have been requesting water all day. Once when I went to get ice water for a patient I was told I am not supposed to be in the area of the ice machine. What a terrible way to spend the last days of your life, there has to be a better option. I could not recommend this facility to anyone.
