Primacy Healthcare and Rehab is an average sized, for profit, nursing home with 120 beds based in Memphis, TN. At last check, the facility had 80 residents indicating that it is 67% occupied which is less than average within the state of Tennessee. The provider accepts only medicare, and provides resident and family counseling services.

My father in law was in this facility for 3 nights during this time he became severely dehydrated and ended back in the hospital. I would not recommend this place to anyone, if you don't believe me go visit! As you walk in the place you will smell feces and urine.The reason being is that they put all the elderly people in diapers because they do not have enough staff to help them get to the port a pot or even a bed pan. Then they leave them in the diapers all day, without checking.All down the hallways are zombies in wheel chairs begging the nurses for help.My father in law was sleeping and a nurse or aid shoved 3 pills in his mouth along with cold water ,startling and frightening him because he was a sleep.My father in law called the place a butcher shop and said people screamed throughout the night. His roommate told us that my father in law would call for help and moaned all night.The staff did not lock the bed of his 378lb roommate so when they were helping him back into the bed he fell to the floor and his bed went flying into our side of the room. Seriously 1 small man helpng a 378lb man into bed?????The rooms and bathrooms are very unsanitary,and Yes I took pictures and videos of the place. We did not know how horrible this place was when the Baptist hospital social worker recommended this place to us!!!!


Response: This is so accurate. My friend is there and I cannot believe the food that is served to an older person. They forgot to give her medicine one day and her blood pressure skyrocketed. She has already had a stroke. The social worker told the family that they could go elsewhere if they did not like it.

Response: Sorry place. would not take my dog there!
