Burlington Health & Rehab is a large, for profit, nursing home with 126 beds based in Burlington, VT. At last check, the facility had 109 residents indicating that it is 87% occupied which is about average within the state of Vermont. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Burlington Health & Rehab, was 3 stars, which is a better rating than 52% of nursing homes in Vermont. Important to note that this facility has recently detriorated and has gone from 4 stars to 3 stars.

I would not recommend this facility to anyone. The parking really is terrible, the place is very dirty(not due to renovations), rooms & bathrooms are not cleaned daily, trash basket in room was over flowing on most of my visits. My dad once spent six days wearing the same clothes. No one changed him. The only good thing I will say about them is, that my dad had a great therpy care.
He was there two and a half weeks before being transferred to the hospital. He picked up an infection that we think he got while at Burlington Rehab.
