Salem Health & Rehabilitation Center is a large, for profit, nursing home with 240 beds based in Salem, VA. At last check, the facility had 196 residents indicating that it is 82% occupied which is about average within the state of Virginia. The provider accepts both medicare and medicaid programs, and provides resident counseling services.

Response: My mother was a patient at the Salem Health & Rehabilitation Center in Salem, VA for eight years. She was treated with the upmost respect and excellent medical care the entire time. Staff always keeps the family informed about your loved one such as medical issues, changing meds, etc. I visited my mother daily and I knew when I left she was always in the best care possible. She always called "Unit 3" her second family. When she passed away in 2010 her obit referred to the Salem Health & Rehabilitation Center (Unit 3)as her second family and we thanked them for their excellent professional care thru the years.

curious as to how dogs and cats are allowed in the facility and how allergies are dealt with in regard to patients and visitors who come and sit in the lobby, etc.

issues with residents passing prescription drugs to other residents, reported to dept of social services, also issues with wound care--bandages not kept on patients properly and skin exposed, patients also leaving the facility without staff knowledge and going across the street to the VA hospital and staff go over there searching for the residents unannounced so as not to allow word to get out so as to diminish referrals from the VA
