York Convalescent Center is an average sized, for profit, nursing home with 60 beds based in Yorktown, VA. At last check, the facility had 56 residents indicating that it is 93% occupied which is about average within the state of Virginia. The provider accepts both medicare and medicaid programs, and provides resident counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for York Convalescent Center, was 3 stars, which is a better rating than 61% of nursing homes in Virginia. Important to note that this facility has recently improved and has gone from 2 stars to 3 stars.

The Medicare Five Star Rating for York Convalescent and Rehabilitaion Center is not correct.


Response: I noticed a lot of incorrect information in this report. The five star rating is incorrect and the state survey date is incorrect. How often is this site updated? If you are not going to update the information, then you should take down your site. It is also very important to publish correct information.
