My mother was there for 6 weeks. Care was terrible. Nursing staff unfriendly, disengaged. Nurse marched in the door grabbed her arm and jabbed a needle in it. I had to ask what she did and she shot out the door saying all residents needed a flu shot. Staff was giving her bananas for a snack when she was being treated dangerously high potassium levels. These are only a couple of things I witnessed.

Father passed away after only 6 months at this facility. Neglect, suspected abuse, reported to state, state did nothing to help us. Sad! Can't even give them a one for a rating. I worry about all the people that are there.

Colony Oaks Care Center is an average sized, for profit, nursing home with 92 beds based in Appleton, WI. At last check, the facility had 50 residents indicating that it is 54% occupied which is less than average within the state of Wisconsin. The provider accepts both medicare and medicaid programs, and provides resident and family counseling services.

Response: I agree .Having had to deal with several of the Kindred locations,the company does not watch what their businesses are doing.It is no wonder they are selling 3 nursing homes in the state.I took my loved one out of here and am glad I now did. It is shameful!

I recently went to visit a friend at this facility that was only there for rehab from a knee replacment.
When I walked into the place, all you could smell from the door was urine. There was no-one around so we had to wander around until we found the desk to find our friends room.
We were for four hours and they didn't even check on her to see if she needed pain meds or check her vitals as she was having problems with her pain levels and her blood pressure. Now I am no medical person, but even I know that if a person is having problems with pain levels, headaches, and blood pressure; you need to keep checking on them.
I was very uncomfortable leaving her in that environment. If I thought I could transport her myself, I never would have left her there.
I would never recommend anyone leaving a loved one in that place. The staff was indifferent, except for 2 young girls only there a few weeks; some rehab people didn't seem to know what they were doing, and there was a general lack of caring for the residents .
I would give it a minus 1 rating. Don't these facilities ever have surprise visits? Shameful.

Same as this person said. The urine odor is so strong I had to leave. Also had the experience of the alarms going off and not 1 person came to turn them off or see if a patient was leaving. We waited to see how long it would take but after 5 minutes went to find help which was another 5 minutes. State needs to take a surprise visit. NOT RECOMMENDED!! Your loved ones deserve a much better place.
