Please do NOT let a loved one stay at Heritage. I had to wheel my mother out the back service entrance at 10 p.m. on a Sunday evening and take her to emergency when the nursing staff did not realize my mother was having congestive heart failure. When I arrived at the hospital with my mother, the nurses on staff were shocked to see someone in such a critical state coming from a "rehab" center. NEVER let anyone you love go to Heritage. The night nurse told my mother to take her medicine and go to bed. She would have been dead within hours! All those nurses know how to do is push pills. It is a terrible place to send anyone.

Please do not allow any loved ones to go to this facility. Most staff we encountered were inattentive, untrained, did not care, blamed others for their poor care and in general provided very poor care. Many promises were made by the management, social worker, Nurse Managers and all failed to do what they said they would do. The staff fall short in all aspects of quality care. If a family member is not there 100% of the time to make sure the basic needs are met, they will not be and your loved one could be at risk of further injury or worse. The only "rehab" that occurs in this facility is that done by therapists on a very limited basis, or what the family provides. This facility would get less than a one star if this were an option!

I guess I have to choose a one star because there is no offering of a zero star. Heritage Square is terrible. The staff has a huge turnaround. The staff is minimal. They are overworked and tend to get short tempered. Patients are freezing, minimal heat in halls and dining room. You wait in the cold hall 30 minutes before being taken into the cold dining room and then another 60 minutes before getting served. I asked the management walking by about the cold conditions and the lack of dining room staff and they just walk by and grumble something and nothing gets resolved. Patients wearing large full sized blankets around their shoulders in order to eat in freezing dining room yet nobody is concerned.

Golden Livingcenter - Heritage Square is an average sized, for profit, nursing home with 100 beds based in Greendale, WI. At last check, the facility had 58 residents indicating that it is 58% occupied which is less than average within the state of Wisconsin. The provider accepts only medicare, and provides resident counseling services.
This nursing home is not located in a hospital or a continuing care retirement community (CCRC). As of November 2012, the medicare rating for Golden Livingcenter Heritage Square, was 4 stars, which is a better rating than 78% of nursing homes in Wisconsin. Important to note that this facility has recently detriorated and has gone from 5 stars to 4 stars.

This place nearly killed my 95-year old mother. Other residents complained about being their and their poor care. My mom had a wound in her leg that was supposed to be cleaned 2 times a day. She ended up in the hospital so incoherent that the doctor that originally saw her had tears in her eyes. They removed two cups of pus from the wound because of the poor care. Call bottons were put out of her reach, she was left alone in the bathroom, her phone wasn't working, and many, many other problems. Her main nurse told us that basically my mother came there to die.
