I filed a comprehensive complaint on this place in 2013. That is when their rating with the state dropped to 1 star. It still is 1 star (because you can't give less than 1 star). I strongly suggest that anyone who has a friend or family member there contact the state of Wisconsin and complain! Just search online for "nursing home complaints Wisconsin". when you file with the state, the feds also receive it. You can even call the toll free number you will find and do the complaint anonymously.
Sounds like nothing has changed since I was stuck there.

They have poor management, all friends with employees, do not know how to discipline when things are done wrong instead they cover up. Resident sits in stool and urine mostly the entire day, residents fall and they do not access or "send to hospital because they don't want to do the paper work or because they don't get paid if a resident go to the hospital". Employees talk on their phone like they're at home, a lot of aids do one round only. The management team we saw giving thumb up in the hallway like little children. Hire staff that cares about what is going on in this facility.

Maintenance and management is horrible. I have been trying to get a light fixed there that is blinking on and off for 2 YEARS outside and it still is not fixed. The place needs new management and new maintenance.

    I was in Souhpointe for several weeks for rehab. I ended up with c diff that's not going away. Medications were missed on a regular basis. Trying to get help from staff was almost impossible. Most of the staff (except a few) were there to collect a paycheck and do as little as possible to get one. The so called therapy is a joke to say the least. I've never met so many rude and unfriendly people in my life. The meals sucked. Only a few good people worked there. My advise-stay away-far away

    So happy to be out of this facility. I loved taking care of my residents but gosh some of the things they put up with in this nursing home from their staff just disgust me. I couldn't stand seeing another good hearted resident being taken care of like crap by some the the staff there. They will start investigations for people who spend a lot of money to be there but not on the ones who aren't "all there" sometimes. I'm sure the resident who sat in her own BM and urine without a brief on for 7 hours, with no food or water given to her and no call light and nothing but a sheet to cover her after getting back from the hospital wanted an investigation done on the aide who left her like that! But because she cant express that she wants to report that aide or that she would like it investigated, they do NOTHING and that terrible aide is still there! Talking on her phone on the unit every single day that I walked in! Or the aide who leaves another resident soaked up to her ear lobes in urine they don't investigate that? Well that explains right there how corrupt this place is. They are more worried about a resident complaining about the head of her bed being down so she can receive proper care then residents being purposefully neglected!
    You wonder why these people are getting bed soars on a regular basis..
    How do you explain two large bed soars on a resident who is continent and doesn't stay in bed?
    Or a stage three bed soar that was "just found"? Can you explain that?
    How about being so understaffed that they leave the dementia unit with one aide and because that aide cant watch 15 people all trying to jump out of bed on a 30 bed unit, a resident fell so much that she started breaking her ribs. Another resident on that floor fell everyday this week. Hmmm...
    I wonder if they know about all of the garbage that place accumulated one weekend. There was so much trash that it filled three dumpsters and 3 separate (x large) bins outside over flowing onto the pavement, and an entire room FILLED end to end within the building. The smell was absolutely revolting.
    I would not give this facility even one star. It is an absolute dump and it should be closed down! I wish all the residents here the best of luck and I will be praying for them.
    If you are reading this review I suggest you keep your family member as far away from this facility as possible!!! If your family member is already there go get them now and take them to a different facility.

    My introduction to SouthPointe was a nurse in charge who said "I am not here to explain the rules. I am here to enforce them." I immediately called a patient advocate and told her I had to leave Counterpoint. She tried but other places were locked down due to the flu.

    From the nurse's unprofessional, anti-patient comments, the place got worse every day. I did file a comprehensive written complaint with the State, feds and the insurance company. Here I will only mention a few.

    I was at the facility for rehab after a knee replacement. For three days after admission I had no physical therapy at all. Terrible for my knee. At no time did anyone ever prepare an individualized care plan for me. No one talked with me about my treatment. Since I had 5 different physical therapists for 10 sessions of PT there was no continuity of care. There was no measurement of my progress. Each session was a beginning session all over again. Useless for me.

    I asked for cold gel packs after PT. I was told over and over again that there weren't any. Other people were using them. Finally a helpful nursing assistant told me where I might find some if they are not all being used. Cold gel packs are necessary to treatment.

    I could not get a shower. I asked over and over again for just one shower after 3 days in the hospital and 2 in the center. I was told that I could only have one in a week after I was admitted to the center. Anti-cleanliness = anti-patient.

    An order was written for me to walk the halls once a day with an assist. One time after a week at the center, a staffer showed up at my door and wanted to walk now. I would not stop the instruction I was being given and tried to set an appointment to walk in the afternoon. The staffer said there was no need for an appointment, she would return. She never did at all. Not any day.

    The administration ignored another chronic medical issue which caused me to be awake too much of the night. I asked for a simple piece of equipment ($150 at Walgreen's) and was told by the social worker: "We don't have any. You'll have to wait." Wait until when? In short, SouthPointe did not provide adequate supplies and equipment that are standard for rehab.

    Other parts of the inadequate "rehab treatment" led me to conclude that SouthPointe is understaffed, too many nursing assistants are working double shifts, necessary supplies are not provided (a helpful nursing assistant told me nursing home residents bring their supplies from home!), administration is unreachable and uncaring (I tried over and over again), and quality patient care is not the goal of the center.

    The rehab was useless and far too expensive. Most importantly, I did not get quality health care with attention to continuity of care. There was never any evidence of any discussion with or without me about my health care. I can, without any hesitation, compare SouthPointe to a third rate motel with meals. This may seem harsh. I have many more examples of repeated failures by SouthPointe providing quality health care. This isn't harsh enough. I tried talking with the Director of Nursing and the center administrator. No success with the administrator and one unresolved conversation with the nursing director.

    This is a review instead of a complaint because I can't think of anything that SouthPointe could do to fix the horrible experience I had.

    Response: Agreed.....they put my husband in an early grave.....bed sores let you sit in diapers for hours.....

    Response: Did the same for my husband. Early grave......bed sores that should have been reported for abuse. Let him sit in soiled clothes for hours. Got a shower only once a week. Wouldn't put my worse enemy into that dump. Surprised it's still operating....

    Wondering who is in charge...mother didn't get her meds in the am because she wanted to go outside. Nurse told me this, I could not believe they did not tell her she has to take her meds before going outside. Also, the right hand doesn't know what the left hand is doing there...was told mom is getting out on 3 different dates.

    There is very little holding this place together. The aids that stay here are either terrible workers OR if they are good workers are getting shafted and just really need the money.

    Under staffed, and poorly managed. I came to Southpointe after being injured and had no idea what I as in for. Have had to wait over an hour at times for response to a care request. The average response is around 5-10. Their food is almost edible, can stomach about 2 meals out off 5.
    On the plus side, the facility itself is clean, and they are at times able to provide good care. Physical rehab staff are top notch and really care for the patients. With some management changes this facility could be very good.
